New & Transfer Students

 

The following information is the responsibility of parents/guardians prior to enrollment. New/Transfer Students will be enrolled when all information is provided and a background check is COMPLETED.

  • Certificate of Indian Blood - If no CIB has been established for child, either parent must provide a copy of their CIB.
  • Birth Certficate
  • Updated Immunization - No child will be admitted without an updated immunization record. For more information visit the NVIC's site.
  • Social Security Card
  • Indian Health Service Consent Form *NEW*
  • Kindergartens - must be 5 years old by December 31st.
  • Copies of Pre-school records for incoming Kindergartners.
  • Official copy of school Withdrawal Form, if transferring from another school district.
  • Copies of Transcripts, Grades, Test Scores, for all transfer students.
  • *Boundary Waiver, if you reside outside the district area.
  • Guardianship Agreement, if child does not live with parents.
  • Copy of Court Documents, Custody Papers, etc, [if applicable]
  • Exceptional Education documents [if applicable].
  • Background Check (by Dean of Student & Registrar).
  • If enrolling in the dormitory, student must first be approved for enrollment with K-8 school.

ENROLLMENT RESTRICTIONS:

Students expelled from prior school :

  • Suspensions/expulsions and/or any other disciplinary action handed down by another school must first be completed and cleared with the last school attended, before he/she is accepted for enrollment.
  • If a student has left another school in lieu of a pending disciplinary action such as suspension, expulsion, or due process hearing, the student may not enroll until an official decision has been made by previous school. Therefore if a student leaves a school under the threat of possible suspension or expulsion he/she may not enroll at Shonto Preparatory School.

 *Out of Boundary Students :

  • Out of boundary students need special approval by their home Agency's Office of Indian Education. Navajo Area out of boundary students require their local School Board Official's approval for enrollment.

Returning Students

Returning students to SPS will be expected to complete the following documents:

  • Completed Returning Application packet.
  • Indian Health Consent form *NEW THIS YEAR*
  • Updated Immunization Card/Record
  • *Boundary Waiver, if you recently moved outside the district area.
  • Guardianship Agreement, if child no longer lives with parents. [if applicable]
  • Copy of Court Documents, Custody Papers, etc, [if applicable]

Printed forms are available at the middle school & front offices, please indicate if your child is returning or new.
